Medical Directors and Officers

Mitchell Ehrlich, MD

Medical Director

Dr. Ehrlich is a specialist in Physical Medicine and Rehabilitation (Physiatry). He has been Board Certified by the American Board of Physical Medicine and Rehab since 1987. He is a graduate of Lafayette College in 1978 with a Bachelor’s in Chemistry and SUNY Downstage College of Medicine in 1982.

Doreen Dimanche-Smith, FNP-C, APRN, MSN, BSN

Chief Medical Officer of Thrive Medical Clinics

Doreen has a paralegal degree and legal studies. Doreen received her BSN and MSN with honors, both from Stony Brook University. She is currently in the process of completing her Doctorate in Executive Leadership. She is a member of the American Academy of Anti-Aging Medicine and the American Association of Nurse Practitioners (AANP). Doreen has experience in pediatrics and adult ICU and emergency rooms. In 2020, Doreen opened Thrive Medical of Smithtown, NY. After successfully managing the office as the President, in 2021 Doreen became the Chief Medical Officer for all the Thrive Medical locations. Doreen is a mother to two step-children and mother of four children. She enjoys being a mom, laughing, shopping, dancing battles with her kids, singing Whitney Houston songs, and spending time with her husband and family.

Medical and Rehab Team Members

Melanie Lentini, FNP-C

Melanie has been in the health care field for over ten years. Her journey began as an LPN. Melanie worked in a nursing home and home health care while she obtained her RN. She attended Stony Brook University for her bachelor’s degree and then went on to study for her master’s at Long Island University for her FNP. She now joins our staff as a certified NP. When she’s not working, she enjoys spending time with her family and friends. Especially her husband, her cat, Chav, and dog Georgia.

Rolande Olivier, NP-C

Rolande Olivier is a board-certified Family Nurse Practitioner; she received her Bachelor’s Degree at Grand Canyon University in Phoenix, Arizona, then went on to get her Master’s in Nursing from Long Island University, graduating as a Nurse Practitioner in 2018. Before becoming a nurse practitioner, Rolande worked with the geriatric population, then continued her career as a trauma nurse in a trauma II center. as a Nurse and nurse practitioner, Rolande has worked in inpatient and outpatient settings. Rolande enjoys writing short stories and reading different genres in her spare time.

Rommel Vargas, PT

Rommel earned his bachelor’s degree in physical therapy from Emilio Aguinaldo College in the Philippines, one of the pioneer schools in the country to offer physical therapy courses. He has extensive experience and post-academic training in the assessment and treatment of general/athletic orthopedic and neuromuscular cases. He works extensively with patients who have orthopedic injuries, chronic pain, neurological disorders, and fibromyalgia.

Acupuncture Team Members

Andrea Conte, MS, LAc, LMT, Dip.Ac(NCCAOM)

Andrea is a board-certified and licensed acupuncturist, as well as a massage therapist. She holds a master’s degree in acupuncture and a bachelor’s degree in professional studies in health and science. In addition, she holds an associate of occupational studies degree in massage therapy.

Jeremy Mierisch, MS, LAc

Jeremy Mierisch, LAC, MS Jeremy was drawn into this field due to being a severe and chronically ill patient for nearly a decade. If it were not for the intervention of eastern medicine combined with the efforts of western medicine, he would not be where he is today. After he got his life back, Jeremy was determined to become a licensed acupuncturist and help as many people as possible.

Chiropractic Team Members

Karen Dion, DC, CCSP

Dr. Karen holds a Doctorate of Chiropractic Medicine from Los Angeles College of Chiropractic and is a Certified Chiropractic Sports Physician (C.C.S.P) from the New York Chiropractic College. She has a Bachelor of Science in Biology from Providence College as well as a Certificate in Nutrition and Kinesiology from the Cooper Clinic in Texas.
